Our History and Experience

Glen Innes Severn Council initiated services for people who are older and people with a disability through Garden Court. Services were then added through the Community Options Project and regional respite programs catering to people of all ages, operating out of Northern Community Care.

In 2014 Northern Community Care and Garden Court collocated under the new name of Life Choices – Support Services to their current location at 134 Church Street, Glen Innes.

If you are older or living with a disability, we can support you to continue living at home. Whatever your situation, we will work with you to ensure the services and support you choose are tailored to your individual needs and enable you to keep living independently and actively in your own home.

NDIS Support Services

We are a locally registered National Disability Scheme (NDIS) provider, with Life Choices – Support Services offering: Coordination of supports, community access (individual and group), daily living activities, capacity building and home maintenance.

Support at Home Program

We provide supports under the Australian Government’s new aged care model, Support at Home (SaH), which is designed to keep people living at home for longer. We can assist older people to have an assessment for funding through My Aged Care. We support independent living by providing in home services, and access to products and equipment that help older people to remain healthy and safe.

Commonwealth Home Support Programme

Commonwealth Home Support Programme (CHSP) for people aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people — who have a referral through the Australian Government’s My Aged Care (MAC) system.

Private Funding Options

Life Choices – Support Services can also provide services for people who aren’t eligible for Government Funding (Private Funding).

Transport & Travel Support

Life Choices - Support Services understand that for some getting around can be challenging. With this in mind, we provide travel and transport assistance to older people and people who have a disability.

Our transport services include:

• A weekly Access Bus provides a door to door service to take people to the Central Business Area so you can do your shopping and attend to business
• Door-to-door transport for special events and local activities
• Examples of this type of service include the Guyra Lamb and Potato Festival, the Glen Innes Show or the Movies

Life Choices - Support Services provides support programs across the Glen Innes Severn local government area (LGA). We proudly serve communities throughout the New England region of New South Wales.
