What are Level 2 Home Care Packages?

Level 2 Home Care Packages assist aged people who are eligible for hostel level care, to remain in their own homes and community.  They are funded by the Australian Government, and give older people a greater say in, and more control over, the design and delivery of community care services provided to them and their carers.

How will a Level 2 Home Care Package help me?

It will provide you with the assistance and services required for you to remain as independent as possible in your own home.

How do I get a Level 2 Home Care Package?

People must first be assessed by the Aged Care Assessment Team (ACAT), who will determine your eligibility.  An ACAT assessment can be arranged by Life Choices - Support Services staff, the hospital, your local Doctor, family or yourself.

What is the Community Options Project?

It is a service for frail aged, people with a disability and their carers, who have complex needs, require case management, and live in Glen Innes Severn, Inverell, Gwydir, Tenterfield Local Government areas or the township of Tingha.

A Support Facilitator can coordinate services for YOU, assist YOU to achieve YOUR goals, supply/loan equipment in special circumstances and broker funds for short-term services if required.

What is the National Respite for Carers Program?

It is a service for carers of older Australians and people with dementia needing a break, and who are living in Glen Innes Severn, Inverell, Gwydir, Tenterfield Local Government Areas or the township of Tingha.

What is the Yetman HACC MSO?

It is a service for frail aged, people with a disability and their carers who live in Yetman and surrounding areas.

Meals and activities are provided at the Yetman Hall every Tuesday: transport, shopping, social outings, companionship Information, and assistance with other services.

What is Aboriginal and Torres Strait Islander Transport?

 It is a service for frail Aged Aboriginal People over 50 years, their carers, Aboriginal People with a disability and their carers who live in Glen Innes Severn, Inverell, Gwydir, Tenterfield Local Government areas or the township of Tingha. Transport options- fuel vouchers, taxi vouchers, Assistance with public and other flexible transport.
